Terms and Conditions: New confirmed referrals will begin to accrue points on June 6, 2011. HH Administrator must approve agency's participation in this program before points can be accrued. New referrals must be submitted and confirmed by September 6, 2011. New referral points are forfeit if referral is canceled by HH or patient refuses therapy. Points are forfeit if not redeemed by October 7, 2011. Prize redemption requests must be made in writing and sent to Maria via fax at 877-436-2233 or email to mgruszka@gemstaff.com. Any HH with a delinquent account at the time of prize redemption has 30 days to reconcile their account before prizes are released, otherwise prizes and points will be forfeited. The assigned liaison therapy coordinator must remain employed with agency in order to receive bonus prize. It is important to note that all prizes will be hand delivered by a GEM staffing representative. Requests for prizes to be mailed will not be honored.
