1. Must be a Licensed PT, PTA, OT, COTA in all 50 US States and Puerto Rico.
2. Two Entries Per Therapist allowed.
3. Stories must be from your own experiences with individuals you have encountered in your career as a therapist (patients, patient families, mentors… etc.) and must solely be the work of the therapist. Plagiarism will result in disqualification.
4. Entries—2-3 page limit, double spaced, font size 10. No more than 500 words.
5. Entries must be received or postmarked no later than August 15, 2011.
6. Gem Staffing has the right to extend deadline extensions when deemed appropriate.
7. Winners will be notified via Phone or Email on October 1st, 2011
8. All Entries become Property of GEM Staffing and will not be returned.
